DENTAL CLAIMS
- Most dentists accept assignment of insurance benefits, so it is normally not necessary for you to file the claim.
- If your dentist does not file insurance claims, he/she will provide you with a standardized ADA (American Dental Association) claim form that he/she has completed to detail the dental work that was performed.
- Submit the claim form provided by your dentist to our office.
- Predetermination of benefits is not required, but it is recommended when charges will total more than $200.00.
