  • How long could you pay your bills without a Paycheck?
    Most people have insurance on their house and car. Unfortunately they often overlook their most valuable asset. Their ability to earn an income. Have you ever thought what life would be like if you could no longer earn an income? Without your paycheck how would you pay for housing, utilities, transportation, food or clothing?

    Disability Insurance helps to provide an income for you when you are Totally Disabled due to a covered Accident or Illness that keeps you away from work for an extended period of time.

  • Here's how your plan works:
    Select the Monthly Disability benefit to meet your individual needs, not to exceed the Maximum monthly benefit for your Monthly Compensation. "Monthly Compensation" means one-twelfth of your annual salary, or, with respect to an Insured whose salary is solely or partially based on commissioned sales, one-twelfth the preceding 12 months' salary. Monthly Compensation does not include bonus or overtime earnings. If there is a reduction of salary that results in you being ineligible for the plan selected, your Monthly Disability Benefit will be reduced to the maximum amount for which you are eligible at the time benefits become payable and any premium overpaid for the previous 12-month period will be returned.

  • Disability Income: Security Plus
    Gold Series Pays a monthly benefit of $___ when you are totally disabled from a covered accident or a covered sickness. The benefit waiting period for accident is ___ days and for sickness is ___ days. Benefits are payable for up to ___ months for a covered accident or a covered sickness. Your payroll deducted premiums are $___ per ___ pay period.
Important Policy Provisions
  • Effective Date of Coverage
    Certificates issued become effective the first of the month following the date of approval of the applicant's written application, provided the first premium has been paid.

    The applicant must be on Active Service on the date his or her coverage would become effective, otherwise the insurance will become effective on the first day of the month following the date the applicant resumes Active Service.

  • Family Accident Benefit Rider. Additional benefits under this Rider include:
    • Accidental Death and Dismemberment Benefit
    • Dislocation/Fracture Indemnity Benefit
    • Medical Transportation Benefit
    • Hospital Confinement Indemnity Benefit
